In order to enroll in any Council of Community Services Insurance plan, you must also fill out an Agency Resolution Form 2008 and a Member Information Form 2008 as well as the appropriate enrollment form.  The most recent copy of your NYS-45 is also required.  Please note that there are seperate offerings for groups of 2 or more.
